Include the following information. • Formal name & applicable contact information • Name and contact information of person authorized to sign contract for the firm • Name and contact information of primary project manager No proposal shall be accepted from, or no contract or purchase order shall be awarded to any person, firm or corporation that is in arrears upon any obligations to the City of Box Elder or that otherwise may be deemed irresponsible or unreliable by the City of Box Elder.
