The County of Riverside Purchasing Department, on behalf of Riverside University Health System – Public Health, Injury Prevention Services (RUHS-PH IPS), is soliciting proposals from qualified consultants to conduct a comprehensive evaluation of Community Health Worker (CHW) services in Riverside County.
This evaluation will assess CHW program outcomes, effectiveness, population health impact, and the contribution of CHW activities to overall service delivery. Community Health Workers (CHWs) play a vital role in advancing access to care, providing health education, supporting care coordination, and connecting residents to essential public health and social service resources. RUHS-PH implements CHW activities across multiple clinic-based and community-based programs, including outreach, chronic disease prevention, injury prevention education, health navigation, and services addressing social determinants of health.
As these services continue to expand, RUHS-PH IPS requires a structured and independent evaluation to assess effectiveness, identify operational strengths, and highlight opportunities for improved coordination and reporting. This project is supported by federal funding awarded to the County of Riverside through the U.S.
Department of Health and Human Services, Centers for Disease Control and Prevention (CDC). The award, issued under Notice of Award Number 5 NE11OE000021-04-00 and FAIN NE11OE000021, is provided through the CDC’s Collaboration with Academia to Strengthen Public Health program.
Funding is associated with the federal project titled “Strengthening Riverside County Public Health Infrastructure, Workforce, and Systems.” The evaluation consultant will work collaboratively with RUHS-PH program staff, including but not limited to, Riverside County Epidemiologist, Hospital Clinic Administration, and Program Coordinator for data review, assessment outcomes, and develop actionable recommendations.
The evaluation will examine how CHW services support community engagement, improve access to care, address social determinants of health, and contribute to improved health and wellness outcomes among priority populations.
Findings from the evaluation will guide continuous quality improvement, inform strategic planning, support future funding opportunities, and enhance the long-term sustainability and impact of CHW services within Riverside County. Funding for this project is not to exceed $75,000, with the anticipated performance period of August 1, 2026 through November 30, 2026, and an option to extend through June 30, 2027.
