Deschutes County, a political subdivision of the State of Oregon, is releasing this competitive solicitation to secure one (1) or more contractors to manage one (1) or more Adult Foster Home (AFH) in properties owned by Deschutes County Behavioral Health. Deschutes County is currently in the process of purchasing and/or renovating two (2) separate homes that are appropriate for use as an AFH. Potential contractors can submit proposals to manage either one (1) or both homes.
Potential contractors would receive below market rent on these properties, but there would not be any county funds paid to contractors to manage the homes. Each home will have the potential for up to five (5) residents. Adult Foster Home Services shall include the operation and management of a licensed, home-like residential setting that provides room, board, supervision, and individualized care and support for up to five residents per home.
Services must promote resident dignity, choice, decision-making, recovery, community integration, and the highest practicable level of independence, consistent with Oregon Adult Foster Home standards and rules.
The Contractor shall provide or arrange for daily supports based on each resident’s assessed needs, including meals and nutrition support, medication administration and monitoring, assistance with activities of daily living, behavioral support and crisis de-escalation, care coordination, resident service or care plan implementation and review, and discharge or transition planning as appropriate.
Services shall be delivered using person-centered, trauma-informed, culturally responsive, and least-restrictive practices, with qualified and trained staff requiring background checks. Contractors will ensure resident rights protections, residency agreements, house rules, emergency preparedness, and all other requirements applicable to licensed Adult Foster Homes in Oregon.
